Cognition
Questions derived from the Mini-Cog test, a 3-minute instrument to screen for cognitive impairment. The Mini-Cog uses a three-item recall test for memory and a simply scored clock-drawing test (CDT). The latter serves as an “informative distractor,” helping to clarify scores when the memory recall score is intermediate.

The STOFHLA is self-administered, and tests a patient’s ability to read passages using real materials from the health care setting. Pages are selected from instructions for preparation for an upper GI series and the patient rights and responsibilities section of a Medicaid application form.

The Incontinence (INC) section asks individuals about their health history related to urinary and bowel incontinence.

The Insurance, Access, and Utilization section provides specific information relating to health insurance and the respondents use of the health care system. The IUQ section was completed during the first interview of a subject at the subject’s home by a trained interviewer.

The occupation (OCQ) section asks questions about the respondent’s work experience. This section goes into details on the type of work that was done and duties performed so that this information can be accurately categorized into occupations.
The Sensory and Dental (SDQ) provides specific information relating to vision, hearing, noise exposure, dental status.

The Physical Activity and Physical Fitness (PAQ) section asks the respondent about physical activities that are done over a typical week. The first series of questions are related to work, then transportation, followed by questions regarding physical activities during recreational time. The last series of questions are related to activities that involve sitting and most questions in this series are asked about the last 30 days. Measured via interview and measured objectively via accelerometry (see accelerometry).
Prescription medications and select OTC drugs (RXQ) provides specific information relating to any medications which respondents may be taking.

The STRESS section measures how often people have feelings and behaviors that might indicate stress, anxiety, and depression. The first part of the questionnaire is based on the Global Perceived Stress Scale, an 8-item self-report measure of perceived chronic stress, originally created for use in the Jackson Heart Study.
